If you want to borrow chemicals that are property of the Thompson Research Group, this form must be filled out prior we lend you any chemicals. The form must be filled out by you (i.e. the borrower), your advisor, and a reference student in the Thompson Group. |

If you want to use equipment in the Thompson Research Group, this form must be filled out prior to the equipment use. The form must be filled out by you (i.e. the user), your advisor, and Prof. Thompson. You must have a meeting with Prof. Thompson to get the form signed.
